There are strict legal guidelines which doctors must follow when issuing Statements of Fitness for Work certificates. You do not need to see a doctor for an illness lasting less than 7 days. The Statutory Sick Pay (SSP) Regulations state that employers are required to accept a self-certification note (completed by you) for the first 7 days of illness. These are available from the Practice or your employer may hold a stock of them. Alternatively you can download a copy and print it off yourself using the following link:
If your illness has lasted longer than 7 days you will need a doctor’s certificate. To order a Statement of Fitness for Work from the doctor we need to know the date it needs to be issued from, the length of time it is needed for and the reason for the request. There are request forms available on the Reception desk.
Please note that we are unable to guarantee that a Statement for Fitness for Work certificate will be issued on request, and you may be asked to make an appointment to discuss your request further
